Registration Packages

Upon acceptance to SKK or SKP, parents will be emailed a registration package to be filled in and returned within 24 hours.

Please fill out all required fields, as your application will not be considered accepted until all information has been correctly recorded, this includes all postal codes, for home, work, doctor, and emergency contact.

​

It is the responsibility of the parent/guardian to report any change of information once enrolled in the program.

Upon acceptance, please notify SKK or SKP if your child requires an Anaphylaxis Form for children with severe allergies.
